§ 14. Commencement of proceedings.

Any five qualified electors may commence initiative or referendum proceedings by filing with the city clerk an affidavit stating their names and addresses; that they will constitute the petitioners' committee; that they are primarily interested in and responsible for the circulation of petitions and that they will file the circulated petition in proper form if sufficient signatures thereto are obtained. The affidavit shall also contain the address to which all notices to the committee are to be sent and the proposed initiative ordinance in full or the citation of the ordinance sought to be reconsidered. Affidavit forms shall be furnished by the city clerk. Without undue delay after the affidavit of the petitioners' committee is filed, the clerk shall issue to the petitioners' committee the appropriate petition blanks.
